We are seeking a compassionate and motivated Care Manager to lead the delivery of the Care Finder Program and provide high-quality case management support to Community Care clients within the Flinders Shire. This rewarding role offers the opportunity to help people navigate the aged care system, connect with essential services, and maintain their independence and wellbeing at home.
As a Care Manager, you will support clients with complex needs to access aged care and community services. Working closely with clients, families, service providers and key stakeholders, you will help individuals overcome barriers, access appropriate supports, and achieve positive outcomes.

Employment status: Permanent Fulltime
Award: The position is classified between a Level 4 - 5 under the Queensland Local Government Industry (Stream A) Award – State 2017
Remuneration: $82,956 - $91,526 gross per annum
ABOUT YOU
The successful candidate will have:
Essential knowledge, skills and abilities
- Experience working with older people or disadvantaged/vulnerable groups.
- Demonstrated knowledge of the relevant funding and supports available for older people.
- Demonstrated ability to understand and apply legislation, policies and procedures, including the ability to identify issues and problem solve.
- Demonstrated ability to take responsibility for work activities and act with initiative to deliver tasks on time, including competing priorities, while working within a team.
- High level of communication and interpersonal skills with the ability to develop effective professional partnerships across diverse stakeholders including vulnerable, aged and frail people and their carers, advocates and government agencies.
- Demonstrated high level of knowledge of the Microsoft Office suite, and the ability to rapidly acquire knowledge of corporate and other programs used by Council such as Authority and TRIM.
- Commitment to continuous improvement and maintaining quality outputs.
- Commitment to uphold Council’s Code of Conduct, Workplace Health and Safety and Anti-Discrimination policies.
Essential education/qualifications/licences
- Current C Class drivers licence.
